The Tenth Planet:

10ep 1-4 – During every outdoor scene in Antarctica, heavy blizzards are shown raging constantly. In reality, such blizzards are extremely rare at the South Pole, thanks to the almost total absence of precipitation (Antarctica is effectively a desert).

20ep 1 – In The War Machines they managed to mis-spell Kit Pedler's surname — now they mess up his first name instead: the writing credit at the beginning of episode 1 refers to him as "Kitt Pedler".

30ep 1,2 – Robert Beatty, a Canadian, was hired to play Gen. Cutler, an American general in the mould of Dr. Strangelove. The script attempts to insert some suitable Americanisms into his dialogue, but the attempt isn't entirely successful — particularly when Cutler assures the astronauts in Zeus 7 that they'll get them down "sure as God made little apples". As any American will know, the actual phrase is "as sure as God made little green apples". Beatty also adds a very Canadian "eh?" to the end of a sentence in episode 1.

40ep 1,2 – On two occasions, General Cutler mistakenly refers to Wigner as "General Secretary" instead of "Secretary General".

50ep 3 – Another credits blunder happens in this episode, where the credits mis-spell Gerry Davis's surname as "Davies".

60ep 3 – As General Cutler is telling Barclay he'll get authorisation to use the Z-Bomb, the camera shakes noticeably.

70ep 4 – In the Radiation Room, Cutler says, "Come on, Barclay, what are we waiting for?" However, it's Dyson who's with him — Barclay is back in the Tracking Room.

80ep 4 – Ben amusingly mixes up his A's and O's when he says, "We save their grotty plonet Mandos, and for what?"
