Doctor Who (1996 TV Movie):

10 The stream of vapour representing the new Doctor's breath is completely out of alignment with Paul McGann's mouth in the brief shot just before he sits up.

20 When the Doctor tells Grace about his ability to regenerate, his voice says "I have thirteen lives" while his lips clearly say "twelve".

30 Grace seems to think that the Doctor's blood is very strange when she looks at it under a microscope, but when we see the probe's view of it, it looks pretty normal.

40 In some shots during the motorcycle chase, it's clearly a stuntman and not Paul McGann riding the bike.

50 Apparently it's impossible to make a "more accurate" atomic clock — there is no isotope with a decay rate as efficient as the caesium istope used in current atomic clocks. Besides, the most accurate chronometers are based on pulsars (or so I'm told).

60 When the Doctor and Grace escape from the Institute by lowering themselves down on a fire hose, notice that there's nowhere near the length needed for the 20-feet-or-so drop that they make.

70 When the Doctor is strapped up in that Clockwork Orange-esque gear at the end, the insides of his lower eyelid are visible on long-range and medium shots, but not on some of the close-ups.

80 When the Earth is sucked into the Eye of Harmony at midnight in San Francisco, the images of Moscow, Paris and London are all also night scenes... when in reality, at that exact moment they would all be in daylight (and no longer celebrating the New Year!) due to the time zone difference: it would be 8am in London, 9am in Paris and 10am in Moscow.

90 During the goodbye scene, the combination of intermittent rain with out-of-order recording results in both the Doctor and Grace's hair changing appearance repeatedly over the course of the scene.
